Nedbod: the BBC is building 162 transmitters for its national multiplex between now and December 2015 which will take coverage up to about 98%. This was agreed as part of the licence fee settlement.
Radio 4 isn't in stereo on FM for 24 hours a day, many of the news & documentary programmes are produced in mono (even though the stereo beacon on your tuner is lit).

Nedbod: The new national commercial multiplex was advertised on 1 July 2014, closing date is 29 January 2015, it's due on air by late 2015. There are at least two known bidders (including one from Bauer/UTV/Arqiva). It's unlikely that the BBC are involved.
Ofcom have allowed 30% of the mux to be used for DAB+, they say they will look at lifting this restriction by 2018.

PJH: If they are on the list they will be on air by the end of 2015. The BBC national mux already has transmitters at Camlough and Newcastle.
There will probably be a phase 5 and I would expect that to include Rostrevor Forest.

Mr David Blake G3MWV: the 700 MHz band is being harmonised for mobile broadband use across the EU so it would be very difficult for the UK to go it alone in keeping it for DTT, due to the potential interference.

Analogue radio listening as a proportion of the total is reducing every year. In Sept 2014 the FM/AM percentage of total listening was 56.4% (down from 59.6% in Sept 2013). Some of this share is going to DAB and some of it is going to online & digital TV listening. The future of radio listening will be a combination of DAB, online and digital TV, but I don't think FM will be switched off before 2024.
At this rate of reduction FM/AM listening will be below 50% of total listening by the end of 2016, and below 25% by 2024.

GrahameP: On Quainton, the BBC national is due by the end of 2015, Herts Beds Bucks local is due by late 2016.

Gary Blackledge: The local multiplex for mid Wales (Ceredigion & Powys) should be on in Brecon by 2016, under a funding agreement signed between the Government, BBC & commercial radio. This will carry BBC Radio Wales & BBC Radio Cymru, as well as some commercial stations.

HuwvThomas: The new North Wales multiplex is launching in December from Arfon & Conwy so there could be some work going on at Conwy in preparation for this.
